Download the Wedding Agreement below, fill it out on your computer or by hand, and email the completed form to the Wedding Administrator. Your date is reserved only once the signed agreement and deposit are received.

Fill it out
Open the PDF and type directly into the fields — names, address, wedding date, and so on — then save it. You can also print it and complete it by hand.

Email it to the church
Open your email (Gmail, Outlook, or whatever you use), start a new message to the address below, attach the PDF you just filled out and saved, and send. Then mail your signed copy with the deposit to reserve your day.
